Case Management Services

Prevention Case Management
Working on an individual basis, the Prevention Case Manager assists individuals who are at high risk for HIV transmission, using a variety of community and educational services, and works with the client to encourage an individual behavioral shift and develop a care plan to meet the client’s needs and goals.

HIV Case Management
Utilizing a wide variety of culturally appropriate services including health education, traditional healing, mental health and social services, the case management team assesses the individual needs of the client, and creates an individualized treatment plan with the clients. Issues addressed in this plan include treatment advocacy, access to medical care and services, substance abuse treatment, and housing.
