Prevention Case Management

Native American AIDS Project’s Case Management Program is an integrated approach to meet the multiple needs of the target population through a culturally specific case management team including case management, peer advocacy, traditional healing, health education, support groups, mental health care and social support services. Our teamwork focuses on client-centered care and encourages access to the spectrum of medical, social, and cultural services available locally, nationally, and inter-nationally. Native American AIDS Project’s care team recognizes the damaging effects that loss of cultural identity can have on Native American individuals and works to encourage culturally appropriate education and reconnection to traditions in working towards individual client’s paths towards wellness.

The Prevention Case Manager assists individuals who are at high risk for HIV transmission due to co-factors such as substance abuse, STD treatment, mental health, social and cultural factors. The Prevention Case Manager encourages client’s behavioral shifts through one-on one sessions, individual risk reduction counseling or support group participation. The Prevention Case Manager completes an intake process with each client who wishes to receive services. Clients will work with the Case Manager to complete an individual service plan which addresses specific issues they would like to work on with the support of the Prevention Case Manager. Additionally, the Prevention Case Manager will review client’s specific issues related to sexuality, condom use/negotiation skills, substance use, domestic violence, housing, and other potential needs. Prevention Case Management also includes prevention with positives, a secondary prevention with HIV+ individuals. Prevention with HIV positive individuals may include individual risk reduction counseling and a review of harm reduction principles related to sex, substance use, and other co-factors.

The Prevention Case Manager works closely with each direct-service staff member through weekly clinical case consultations and individual meetings to ensure that our community receives complementary services to help them reach their goals. All clients are encouraged to check in regularly with members of the case management team with the understanding that frequency of contact increases the case management staff’s ablilty to more accurately asses client’s needs. The Prevention Case Manager may also engage clients in linkages and navigation of programs with other service-providers including: medical, housing, tribal government services, disability, and legal services.
