Native American AIDS Project’s Case Management Program is an integrated approach to meet the multiple needs of the target population through a culturally specific case management team including case management, peer advocacy, traditional healing, health education, support groups, mental health care and social support services. Our teamwork focuses on client-centered care and encourages access to the spectrum of medical, social, and cultural services available locally, nationally, and inter-nationally. Native American AIDS Project’s care team recognizes the damaging effects that loss of cultural identity can have on Native American individuals and works to encourage culturally appropriate education and reconnection to traditions in working towards individual client’s paths towards wellness.

The HIV Case Manager provides individual needs assessment, legal assistance, housing, food, clothing, benefits counseling, community activities referrals for support groups and therapy for clients. The client and HIV Case Manager work together to develop an individual care plan that outlines goals, objectives and activities designed to meet the client’s needs and preferences for services and support, including treatment education and advocacy services. The HIV Case Manager encourages client’s behavioral shifts through one-on one sessions, individual risk reduction counseling or support group participation. In addition, the HIV Case Manager provides in-depth client education and support for HIV positive individuals in areas such as treatment adherence, medical service navigation, and increasing client understanding of lab results.

The HIV Case Manager provides assistance in obtaining benefits such as ADAP, Medi-Cal, Social Security, and other state and federal benefits. The HIV  Case Manager also conducts numerous service linkages and assists clients in navigating a wide array of supportive services including medical, housing, and substance abuse treatment programs. The NAAP HIV Case Manager is particularly adept in conducting housing referrals and follow-ups in order to best assist and support clients in obraining emergency and/or long-term housing. In addition, clients, their partners and family members may receive individual risk reduction counseling.
